Welcome to Ultimate Health Check. These Terms and Conditions govern your use of our services, including the purchase and use of diagnostic test kits and related health consultations. By placing an order or using our services, you agree to these Terms.

 

1. Services Provided

Ultimate Health Check offers bespoke health assessment services including:

  • Provision of at-home diagnostic test kits from accredited laboratories.

  • Initial and follow-up health consultations.

  • Preparation of personalised health reports, treatment plans, and recommendations.

  • Communication and liaison with laboratories on clients’ behalf.

 

2. Orders and Payment

  • Orders can be placed online or by other approved methods.

  • Full payment is required before services and test kits are dispatched.

  • Prices include VAT where applicable and are as stated on the website or in invoices.

 

3. Refund and Return Policy

3.1 Refund Eligibility

Refunds are only available under the following conditions:

  • A refund can be requested within 14 days of delivery of the test kits.

  • All unused test kits must be returned to the respective laboratories within 14 days of refund request.

  • The Defendant will be charged by laboratories for kits used or processed; hence refund is conditional on return.

3.2 Non-Refundable Items

  • Consultations, reports, questionnaires, and health advice or treatment plans cannot be refunded as these services are provided immediately upon purchase.

3.3 Return Process

  • Test kits must be returned using trackable shipping. Customers bear return shipping costs unless error is on our part.

  • Upon receipt and satisfactory inspection by laboratories, refunds will be issued within 14 business days via the original payment method.

3.4 Kit Transfer

  • Customers unable to perform tests themselves may transfer kits to a third party. Notify us of any transfer for coordination with laboratories.

3.5 Refund Denial

Refunds may be denied if kits are used or not returned, are damaged, or if the return timeframe is missed.

 

4. Customer Obligations

  • Customers must provide accurate details and complete any required questionnaires promptly.

  • Customers agree to return test kits within the stated timeframe if requesting a refund.

  • Customers acknowledge that test kit usage and delivery procedures must adhere to local laws and laboratory requirements.

 

5. Limitation of Liability

  • Ultimate Health Check is not liable for delays or loss in shipping of returned test kits.

  • Liability is limited to the cost of the goods and services provided, excluding incidental or consequential damages.

  • We make no guarantees regarding diagnosis or treatment outcomes as tests are interpreted in context of professional medical advice.

 

6. Intellectual Property

  • All proprietary information, methodologies, questionnaires, reports, and communications remain the intellectual property of Ultimate Health Check.

  • Unauthorised use or reproduction is prohibited, and breach may result in legal action including claims under the Fraud Act 2006.

 

7. Privacy and Data Protection

  • We collect and process personal data in accordance with applicable data protection laws and our Privacy Policy.

  • By using our services, you consent to this process for the purpose of providing the contracted services.

 

8. Communications and Conduct

  • We expect respectful communication. Harassment, abusive language, or threats may result in termination of services.

  • We reserve the right to take legal action if communications cause significant disruption or loss.

 

9. Governing Law and Jurisdiction

  • These Terms and your use of our services are governed by the laws of England and Wales.

  • Disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.

 

10. Changes to Terms

  • We reserve the right to update these Terms and Conditions at any time.

  • Updates will be posted on the website with an effective date; continued use constitutes acceptance.
